To hire an insurance provider for the City of Brampton’s Employee Group Benefit Program. The program includes: Group Health, Dental, Vision benefits and the Life Insurance, Long Term Disability (LTD) and Accidental Death and Dismemberment Insurance (AD&D) for employees from Non-union, CUPE Office / Outside / Professional and Technical, ATU-Transit, BPFFA-Fire Department, Elected Officials, Retirees and Employees on disability leave with Direct Benefits, Community Groups and Contract.

This procurement is subject to CETA and CFTA.

 Non-Disclosure Agreement (NDA) Appendix F: In order to assist the Proponents in submitting a Proposal, the following confidential information shall be made available to all Plan Takers and Proponents who return a completed and properly executed NDA, as supplied in Appendix F of this RFP.

1) Appendix E: Disabled Member Listing and

2) Appendix G: Census Data

Electronic (Scanned/PDF) copies of the completed NDA must be returned to the following email address: saj.thomas@brampton.ca. Modifications to the NDA will not be considered. Once a completed and properly executed NDA is received, the City of Brampton will provide a password protected Appendix E: Disabled Member Listing and Appendix G: Employee Census Data, via email.
